A 26-year-old Sioux Lookout man is facing 5 charges after a single vehicle collision last weekend.
Officers were called after a pickup truck crashed into a telephone pole around 10:30 Sunday morning.
Police say Mike Janvier has been charged with impaired driving, refusing to provide a breath sample and 3 counts of breach of probation.
He has been released and will appear in Court on September 21st.
The accused was not injured in the incident.
