Helping those living with diabetes in Northwestern Ontario.
That’s the focus of the Northwest Local Health Integration Network, as officials look at the current state of diabetes in the region.
Senior Director of Health System Transformation Susan Pilatzke says their main focus is improving the coordination of care.
She says they want to make sure the patient living with diabetes can move across the health care system in a timely fashion, have access to the care that they need and have them manage their diabetes successfully.
Over the next year, the Northwest LHIN will meet with diabetes care providers across the region and coming up with a plan to start to coordinate the care differently.
In 2012, the LHINs took over the role for diabetes services from the Ministry of Health and Long-Term Care.
Northwestern Ontario has one of the highest numbers of people living with diabetes in the province.
