The Home and Community Care Nurse/Community Health Nurse (HCC/CHN) provide services to Wabigoon Lake Ojibway Nation, and Wabauskang First Nation located within the Treaty #3 area.  The HCC/CHN will work with the community Health Director of each community and reports to the Health Manager of Paawidigong First Nations Forum Inc..  The HCC/CHN delivers core community health services to residents of the community.

SUMMARY OF DUTIES AND RESPONSIBILITIES:

 

  • Provide Home Care and case management services.
  • Continually assessing the client’s health status.
  • Consulting with the client’s physician/NP or other health agencies/professionals.
  • Maintaining client records.
  • Maintaining statistical reports.
  • Maintaining client confidentiality.
  • Providing support to the client/family by acting as a resource/advocate, i.e. medical supplies, and equipment.
  • Providing teaching to client/caregivers.
  • Provide footcare support to our Diabetes Strategy Nurse
  • Providing Education through presenting
  • Providing communities with current education and awareness on chronic disease prevention

 

SUMMARY OF KNOWLEDGE & SKILLS:

 

  • Registered Nurse with a Bachelor of Science Degree in Nursing
  • Must be registered with the College of Nurses of Ontario.
  • Minimum of one-year experience in Community Health Nursing.
  • Cultural sensitivity and experience working with First Nations.
  • Required to posses/obtain certification in Advanced Footcare Program
  • Ability to work independently and as a team member.
  • Demonstrated oral and written communication skills.
  • Demonstrated supervisory, leadership, managerial and computer skills.
  • The ability to speak/understand the Ojibway language is an asset.
  • Travel is a requirement of this position therefore, Valid Driver’s License and access to a reliable personal vehicle is required as well as clean driving abstract
