Coordinate substance use services, supporting appropriate access and continuity between hospital and community care.

DRHC is currently recruiting a Permanent Full-Time Substance Use Disorders Case Manager to join our Mental Health & Addictions Services team.

This role provides assessment, brief intervention, crisis support, and coordinated care to individuals experiencing substance use concerns. Working closely with the Emergency Department and interdisciplinary teams, this position supports individuals from initial presentation through discharge and into the community, helping improve access, continuity, and outcomes.
