Improving care and services at the Dryden Area Family Health Team through a new Quality Improvement Plan.
Dryden Regional Health Center CEO Wade Petranik says this is the first year the health team will use it, and they will work closely with the hospital.
The hospital has the overall governance for the health team and the hospital, so Petranik says they will be able to use the existing structures and talent in place to further implement the plan.
Some features in the plan include timely access to primary care, primary care visits post discharge and improving the general patient experience with the use of a survey.
This the first time the health team will be following a QIP, but Petranik notes they have already collected statistics and data from previous years and followed a similar model.
The hospital has followed a QIP for the past three-years.
