LHIN Looking at Chronic Health

A new strategy is being developed by the Northwest Local Health
Integration Network to improve chronic disease prevention and management.
Spokesperson Mike Hindmarsh says patients manage their chronic
conditions independently, so it’s important that health care providers
support them.
Hindmarsh says the new strategy includes assessment, goal setting,
action planning, problem solving and follow up.
Workshops with health care professionals were recently held in Dryden
and Thunder Bay.
