A new strategy is being developed by the Northwest Local Health
Integration Network to improve chronic disease prevention and management
strategy’s.
Spokesperson Mike Hindmarsh says patients manage their chronic
conditions independently, so it’s important that health care providers
support self-management for their patients.
Hindmarsh says its a Ministry of Health led initiative.
