A senior Alberta social worker has been fired for her part in the case of twin baby girls who died in Thunder Bay while being driven to Southern Ontario.
A review panel found the social worker failed to ensure the girls would be safe while being driven across the country by their grandfather.
The twins died in a Thunder Bay motel room after being stranded in a freezing car for three hours.
Their grandfather is charged with criminal negligence.
The children had been seized from their teenaged parents in Slave Lake, Alberta.
